Complete a Personal Change form to ensure your name is up-to-date.

Requirements
Forms and Documents
To change your name send MEPP a completed Personal Change form and a certified copy of ONE of the following:
- Change of Name certificate
- Birth certificate
- Divorce certificate or Decree Absolute
- Marriage certificate
- Spouse's Death certificate
If your name change is a result of a spousal relationship change, please see the Spousal relationship change form page as additional documents may be required.
Refer to Acceptable Documentation, for information on how to certify your documents and what document(s) you may need to change your information.
Processing Time
MEPP will process your name change in approximately three business days of receipt of all required documents.
Fees
There are no fees to change your name with MEPP.
